Given a registry, abstract information from a health record and correctly enter it into the registry.
Back

I learned that Orchard Park does not contribute to any registries.  When the patients get to the facility, they have already been diagnosed at the hospital.  For example, patients may have cancer, but they are diagnosed previous to residing at Orchard Park.  They do not provide cancer treatments or necessarily document the specifics of the disease, so they do not add to the cancer registry.  I did, however, have a lot of opportunities to abstract information from the health record and enter it into databases, similar to that of entering in information to registries.  I have described several of these in depth in other areas of this site, but I will give a broad overview of some instances.

  • I abstracted information from newly admitted patients and recently discharge patients into an online quality improvement system called AbaqisFor more information on using Abaqis, click here.

  • I abstracted information as I logged new admits and discharges into the log book, called the Admission/Discharge RegisterFor more information on the log book, click here.

  • When creating the Physician's Exam List, I had to abstract information from NetSolutions where the patient's most recent vitals are recorded.
